Community Health Needs Assessment

Every three years, Hills & Dales General Hospital completes a Community Health Needs Assessment (CHNA). Our goal is to learn the most pressing health needs of the communities which we serve and then develop plans to address those needs. We’re in the process of completing our 2019/2020 CHNA and we need your help.  Please take a few minutes and complete our CHNA survey.  You can do this at any Hills & Dales location in Cass City, Caro, Ubly or Kingston.  You may also take the survey online at https://www.surveymonkey.com/r/HillsandDales2019CHNA.
